An Arlington County Commonwealth Coordinated Care Plus (CCC+) Town Hall Meeting will be held at Central Library Auditorium on Monday, November 13 from 10:30 a.m. to 12:30 p.m.

What is Commonwealth Coordinated Care Plus?

Commonwealth Coordinated Care Plus is a new statewide Medicaid managed long term services and supports program that will serve approximately 214,000 individuals with complex care needs, through an integrated delivery model, across the full continuum of care. Enrollment into CCC Plus is required for qualifying populations. Care management is at the heart of the CCC Plus high-touch, person-centered program design. CCC Plus focuses on improving quality, access and efficiency.

Who Should Attend?

This program is intended for Medicaid members, caregivers, families and advocates, as well as anyone else seeking a better understanding of the provision of Medicaid benefits.

About the Event

The event will consist of a presentation by the Department of Medical Assistance Services (DMAS), and all six of the Managed Care Organizations (MCOs) that are participating in this new Medicaid Managed Care initiative which begins in Northern Virginia on December 1, 2017.

Katie Hill and Lynne Vest of the DMAS CCC+ Outreach team will begin the presentation with an overview of the entire program, it’s background and implementation, followed by descriptions of each of the six MCO health plans:

Aetna Better Health of Virginia
Anthem HealthKeepers Plus
Magellan Complete Care of Virginia
Optima Health
United Healthcare
Virginia Premier Health Plan
Outreach team members from the health plans will then make a brief presentation on their plan.

The presentations will be followed by a question and answer session where plan beneficiaries, their care partners and family members will have the opportunity to ask both DMAS and each of the plan providers questions about how this new managed care coverage will be implemented so that they may decide for themselves which of the six available plans will best meet their needs.

Who Will Participate in Commonwealth Coordinated Care Plus?

CCC Plus includes Medicaid members who:

Receive Medicare benefits and full Medicaid benefits (dual eligible), including members enrolled in Commonwealth Coordinated Care (CCC).
Receive Medicaid long term services and supports (LTSS) in a facility or through one of the home and community-based (HCBS) waivers, except Alzheimer’s Assisted Living waiver. Individuals enrolled in the Community Living, the Family and Individual Support, and Building Independence waivers, known as the Developmental Disabilities (DD) waivers, will enroll for their non-waiver services only. At this time, their DD waiver services will continue to be covered through Medicaid fee-for-service.
Are eligible in the Aged, Blind, and Disabled (ABD) Medicaid coverage groups, including ABD individuals currently enrolled in the Medallion 3.0 program.
