With hands-only cpr and user friendly defibrillators in public places.

Here to help us feel more comfortable with lending a hand in an emergency is ems, lieutenant nicole nappi,, and she has brought a friend.. Thank you so much for joining us. (laughs). Well, thank you. Ellen,.

Thank you for having me here, today.

A

Sure, so tell us a little bit about how hands-only cpr,, I believe you call it,.

A

Has made it a lot easier. I do remember, in high school,. You know we all went through cpr training, and it was a little bit complicated and intimidating.

B

It was a little complicated and intimidating because you had to remember so many numbers.. Now it is just very simple,.

B

You just have to remember where your hands have to be placed, and to push hard and fast to make sure that we circulate that blood.

B

push hard and fast and just keep on pushing?.

A

Mm-Hmm. now, describe a situation where a good samaritan or a bystander could step in,.

A

And kinda what they should do, what they should do. First?.

B

Well, first thing you wanna do: is you wanna make sure the patient is not responding. so,? If you have a patient that has collapsed.

B

Approach the patient, kind of tap them,, see if they're responsive..

B

If there is no response, and you don't see any signs of life,.

B

You don't observe any type of breathing, at that point, we can get into cpr..

B

If you know how to check a pulse,, you can certainly check a pulse.. If you don't,, we can get right into cpr.

B

so,. It is that simple. okay,. But if you see.

A

If you see somebody who's collapsed and you check if they're responsive,, I'm thinking, regardless,, you still wanna call 911 immediately.

B

Oh, absolutely, absolutely yes,, yes, yes.

A

And then, what you're talking about,? Let's say that the person is lying here, and they are breathing, and they're in distress.

A

Then that's not time to do cpr, right?.

B

Absolutely not, no. just coach them. contact 911, stay with the patient.

A

And then the 911 operator will tell you what to do.

A

Until the ems folks get there.

A

yes,, yes, they'll stay on the line with you,.

B

They'll, give you instructions on what you need to do for the patient until they arrive.

A

Alright, but so people shouldn't be intimidated to go ahead and jump in and try to help..

A

So, let's say, wherever I am,, maybe I'm out in the park and I've come upon somebody who's. Laying there.

A

And not responsive.. What do I do?.

B

Well, one of the first things you want to do,, like I said,. If you know how to check for a pulse, you could., first and foremost,, call 911 or get someone to call 911..

B

We have an aed here, today,, which we can show you how to use.. If you have a bystander around,, you can call and see. If there is an aed available,.

B

But we're going to show you how to do hands-only, cpr today.

A

Okay,, that's great. and tell me more about what an aed is, and where they're located in baltimore county. okay,. So an aed is an automatic external defibrillator.

B

It sends a shock through the heart..

B

The attempt is to restore a regular heart rhythm.

B

For those rhythms that are not viable for the patient.

B

They will literally walk you through everything you need to do. so,, you don't even have to think about it..

B

All you need to do is turn the aed on.

A

And it'll walk you through everything. You need to do.

B

Alright, cause, you think, of, you know,.

A

The shock pads and all, you think of them, you know, on television, in the hospital shows, in the er.

A

But what you're saying is that these are out in the community, and its got some sort of a computer in there that talks to you and tells you what to do.

A

everything, tells you everything you need to do.

B

Well, let's get started. even shows you.

A

Where the pads need to go.

B

A

Alright,, so I'm at the park,. Let me stand up here,.

A

And I've come upon somebody and they probably have clothes on.. Do I have to open up the, the clothes?.

B

Yes,, so you should open up the clothes,, especially if you have an aed available,, because these pads need to have complete skin contact.

B

Okay,, so that is very important., so I'll be your bystander,.

B

And I'll go go, get the, I'll, go call 911 and I'll get the aed for you.

B

While you start cpr.

A

But meanwhile, I say: "are you okay,! Are you okay?" no,? No answer.? I don't feel any breath.. I think "oh, my god,, this person's in cardiac arrest." yes,, so you're gonna place your hands.

B

On the center of the chest, lower half of the sternum. alright, and I'm doing sort of getting the heel of my hand.

A

Together, here?, the heel of your hand,.

B

Right in the center of that chest. alright, right, right between the nipples.

A

Is essentially yep, exactly., where you're saying?.

B

So your goal: is, you're, looking at 100 to 120 compressions, a minute.

B

[ellen], that's a lot., that's a lot,, so you're pushing pretty fast.

B

and you want to, the goal,. The thought in your mind is to circulate that blood throughout the body. cause. Blood carries oxygen,, so we're trying to perfuse those organs, and we're trying to feed them. okay,. So I'll give.

A

This a try and anybody, you know, if you're watching,, this is basically instruction on how to do this,.

A

You don't necessarily have to go, get a cpr class,.

A

This is telling what to do.. This is basic instructions. alright,, so here goes.

B

(Pumping) I just start. okay,, so I'm gonna come in,. I called 911, so they're on their way and I'm gonna get the aed ready..

B

So the first thing I'm gonna do, I bring it up,, I'm gonna turn it on..

A

And by the way, [aed] remove clothing. How hard am I supposed to be pushing?.

B

You wanna get down at least two inches,, so it's...

A

Oh my gosh, [aed] open packet. I wasn't doing it enough. and apply pads onto patient's bare skin. okay, come closer.. Gotta push a little further, gotta come closer, a little harder. come closer. wow,, okay,, that's, [aed]apply pads, that's pretty strong.!.

C

[Aed], as shown in the pictures. okay,, so the aed just told us to open up the shirt.

C

[Aed] open packet and apply pads onto patient's bare skin. and they're, telling us to open up the package.

B

-[Aed] apply pads and, as you see here,.

[Aed], as shown in the pictures.

The aed pads actually tell us where to put the pad.

B

open packet and apply pads, so I'm gonna work around you. onto patient's bare skin.. You continue to do cpr.

A

Because it's not electrified right, now? [aed] press pads, firmly on skin. nope. alright.

C

(Tearing) [aed] apply pads, as shown in the pictures. okay,, so you're placing [aed]press pads firmly on skin.

A

It on the upper part of the right? yep. and actually below where the heart is on the left. yes. do not touch the patient.

B

Okay, now I'm gonna ask analyzing heart rhythm.

C

You to step off, because it's analyzing, the rhythm.

A

Step back,, right,, yep.

C

[Aed] do not touch the patient, analyzing heart rhythm.

C

A

So it's doing it right. Now? [aed] do not touch the patient.

B

Yep, it's reading, the, it's...

A

So it just assessed the patient.

B

[Aed] (beeping) press the flashing shock, button. and figure out that... so now, when this flashes, [aed] press the shock button now. I'm in control of pushing that shock button,. So I'm gonna,, I'm clear, you're, clear, press the shock button, now. everybody's clear.

B

and I'm gonna push that shock. Button.

C

[Aed] shock delivered. and this patient would've jumped.

B

Begin cpr. and I want you.

A

To get right back on that chest and start cpr. oh,, I jump right back in?. As long as the light is off? yep. okay., (pumping) [nicole] we're gonna have you... [ellen] whoops,, I forgot. [nicole], but it's more important [aed] 10 [nicole] just to push and keep pushing. [ellen] okay. (pumping).

B

[Nicole] yep, and we're gonna continue cpr. [aed] 20. Now this aed will count down two minutes, okay?.

C

30, give two breaths. and every two minutes it will re-cycle,.

B

I'm not sure if you can hear me over the aed,, but every two minutes, it will re-cycle and reanalyze that heart, and it will give one shock every two minutes. [ellen] and I gotta keep going. you do, and hopefully, somebody else,.

B

If we get the word out, there, [aed] 10., somebody else knows cpr and they can jump in and give you a hand. or even if.

A

We're just instructing them here,.

B

A

You put your hands here and just push,. They don't have to have specialized training.

B

Absolutely,, I'm gonna shut this down, so we can...

B

A

Are we back, we're alive,, we're good?? We could be. okay. and if not, we could be, hopefully.. If not, do you just keep pushing?.

B

Absolutely, yes. until 911 arrives, or if you're too, tired to continue.

B

But keep the thought in your mind, you're trying to save this person, so...

A

Yeah, and if there's somebody else,, you know,, you can pull them in. If you really are.

B

And instruct them how. exhausted.

A

Yeah,, but for me what's really interesting is that it is so much simpler than what we learned before.

A

It is, it is. and it's just,. Take your hands like this,, go in the middle of, between the nipples and the middle of the chest, and kind of have your hands there,, and your arms straight, and just push and keep on pushing.

A

(pumping). It's pretty easy,. I think you have it. alright,. What else should people know.

A

If they come upon an emergency situation like this?.

A

Anything special, just kinda...?.

B

You definitely want to contact 911, immediately.

B

A

Yep, do not delay.

B

911, they are instructed to give you instructions,.

B

So, even if you are afraid, or you don't know how to do cpr or you forgot something,, they can certainly walk you through the process.

B

okay, well, thank you.. So it don't be...

So,, what I'm hearing, the biggest point is: go ahead and jump right, in,, engage, call 911,, don't be afraid to do the cpr.

A
