This is an instructional video designed to provide applicants assistance with how to complete the required W-9 form, to ensure success submitting an application to the COVID-19 Baltimore County Small Business Emergency Relief Grant Fund Program. This video will assist those who have submitted an application that included a W-9 that was not accurately completed. In an effort to help Baltimore County businesses as much as possible during these trying times, we are allowing these applicants to resubmit a new, accurately completed and signed W-9.

Development.- I'm here today to provide instructions on how to complete the required w-9 form, to ensure success in submitting your application to the baltimore county, covid-19 small business emergency grant fund program.

You're most likely watching this video because you submitted an application, that included a w-9 that was not accurately completed.

in an effort to help our businesses as much as possible,. During these trying times., we are allowing you to resubmit a new w-9.

So, let's just jump right, in.

The w-9 is an irs form that allows baltimore county, to validate the existence of the business or individuals seeking financial assistance..

A

The w-9 must be completed accurately or your grant request will be rejected and funding will not be provided..

A

This video and the email you received, from baltimore county, provide both written and verbal information to increase the likelihood,. You submit an accurate and complete w-9.

in the following slide,. We will walk you through each block and the corresponding information.

Line one should match the legal name on your tax return for the business or individual, applying for the grant..

It is important that this information be an exact match, to the information you provide on your tax return..

You must enter one of the following on line. One and remember, do not leave this line blank.

you may enter as an individual,, which generally enters the name shown on your tax return..

A

If you have changed your last name, without informing the social security administration of the name, change, enter your first name,, the last name as shown on your social security card and your new last name.

A

or you may enter as a sole proprietor or single member llc.

A

enter your individual name,, as shown on your 1040, 1040-a, 1040-ez,, on line one.

A

You may enter your business trade or doing business as name on line two.

A

Or you may enter as a partnership, in llc, that is not a single member, llc, c corporation, s corp or s corporation.

A

Enter the entities name as shown on the entities, tax, return on line one and any business trade or dba name on line two.

A

If you file your taxes as an individual,, but you also have a business, name, trade, name,, dba name or disregarded entity name,, you may enter it on line two..

A

Your legal business name, must be entered on line one.

A

Check the appropriate box on line three for the us federal tax classification of the person whose name is entered on line one.

A

Check only one box on line three.

A

Additional information can be found in the instructions in the email you received from baltimore county.

Line four must only be completed if applicable.

A

Lines five and six are very straightforward..

The address should reflect the appropriate address for the entity or individual applying for financial assistance..

A

Baltimore county prefers a physical address over a post office. Box.

A

The taxpayer, identification, number or tin is necessary to ensure the entity exists and is properly registered with the irs.

A

in this section the applicant will either enter in your social security number or your employee identification number..

You will not have information in both of these boxes.. This must correspond, to the information on the tax return for the person or entity seeking financial assistance..

You absolutely will not be eligible for this grant. If this information is incomplete or inaccurate.

To complete the form,, the applicant must complete part two of the w-9 certification.. The signature should be from a person that is authorized to sign official documents on behalf of the entity or individual seeking assistance.

And finally, the next steps, the applicant should accurately and completely fill out a new w-9, by utilizing the link in the email you received from covidbizrelief@baltimorecountymd.gov.

You will then compose an email, to covidbizrelief@baltimorecountymd.gov.

A

that must include the applicant's unique id corresponding to the submitted application, that was provided during the initial process and the accurately completed w-9.
