boston
community
health
needs
assessment
or
chna,
and
the
community
health
improvement
plan,
which
is
chip
collaborative,
have
awarded
this
grant,
which
will
fund
a
citywide
community
health
needs
assessment
and
community
health
improvement
plan
process.
That
plan
would
provide
a
detailed
roadmap
of
achieving
racial
and
ethnic
health
equity,
with
recommendations
for
action
and
partnership
on
each
of
these
priorities,
those
priorities
being
housing,
affordability
and
access,
economic
mobility,
mental
health
and
behavioral
health
and
accessing
services.

Last
year
I
declared,
along
with
the
city
of
boston,
racism
is
a
public
health
crisis,
and
so
this
certainly
seems
like
it
would
go
towards
addressing
sort
of
the
concerns
that
the
data
and
equity
issues
that
we
saw
with
that,
and
so
with
that
introduction,
I'm
going
to
kick
it
over
to
mata
rivera
to
give
a
little
introduction
and
then
answer
a
question
or
two
and
then
I
believe
will
be
concluded.

This
fits
squarely
into
that
and
I
think
there
is
I'm
not
sure
if
someone's
going
to
pull
up
the
slide
deck

if
I
should
pull
it

as
you
explained
councilman,
thank

You
did
that
very
well
that
I
could
have
the
ship
and
the
chimney
are
part
of
the
process.

The
work
process
that
is
expected
of
the
community
health
centers
and
the
hospitals
and
and
local
board
of
health
and
public
health
commission,
and
through
this
needs
assessment
they
in
this
process.
They
essentially
determine
what
the
needs
are
in
the
area
and
they
come
up
with
a
plan
for
how
to
address
those
and,
historically
and
typically
what
they
do.

Is
they
do
this
individually
and
several
years
ago,
as
you
can
imagine
several
hospitals,
however
many
we
have
in
boston
and
our
local
board
of
health
throughout
the
years
have
done
this
individually,
and
you
know,
essentially,
the
community
residents
and
ceos
after
having
to
engage
in
so
many
focus
groups
and
surveys.

It's
it's
exhausting
and
they
several
years
ago,
in
2017,
this.
B
They
decided
to
come
together
and
do
one
process,
and
so
they
formed
the
boston
collaborate.
The
boston
chinooks
are
collaborative
and
I'm
gonna
go
through
this
quickly,
but
I'm
not
giving
you
this
process.
I
apologize
for
all
the
people
that
put
so
much
time
and
effort
into
this.

There
is
a
website
that
that
gives
a
little
more
background
and
and
talks
about
the
principles
and
values,
and
I
encourage
people
to
look
at
it
because
there
was
so
much
work
that
went
into
it
and
the
boston
collaborative
had
a
set
of
principles
and
they're
outlined
and
include
cbo's
stakeholders,
community
health,
centers,
hospitals,
and
so
through
that
process.
Again
they
came
together
and.

And
so
once
the
the
community
health
needs
assessment
was
was
completed
and
the
four
priority
areas
that
resulted
that
again
you
mentioned,
were
identified,
the
they
embarked
on
from
those
areas
developing
the
the
chip
and
so
I'm
attracting.

2019
and
then
they
released
an
rfp
and
were
looking
for
an
organization
to
essentially
implement
the
chip
and
that's
how
we
got
to
hhs
getting
the
grant
to
implement
the
chip
and,
of
course
the
pandemic
has
sold
that
timeline.

So
by
now
we
should
have
been
in
the
place
where
we
would
be
implementing
the
plan
and
have
hired
staff
and
we're
hoping
to
be
able
to
catch
up
on
a
timeline,
and
so
that's
what
this
grant
is
essentially
is
to
be
able
to
hire
staff
to
implement
the
plan
that
came
out
of
this
process.
That
has
been
you
know
several
years.

The
role
of
hhs
in
this
is
gonna,
as
I
mentioned,
that
we'll
hire
a
couple
of
staff
that
are
gonna
be
obviously
boston.
Employees
they'll,
be
working
with
hhs
and
hhs,
in
collaboration
with
the
syrian
committee,
will
on
a
high
level
work
to
derive
the
plan
implement.
It
will
also
be
part
of
the
student
committee
as
well
as
this
public
health
consumer
continuity
on
the
health
congress,
student
committee
of
the
collaborative
and
we'll
provide
guidance
on.

Those
are
the
ud
partnership.
Is
that
having
this,
this
hhs
as
the
backbone
provides
an
opportunity
for
broader
reach
with
the
church,
broader
accountability,
because
we're
able
to
do
all
of
our
departments,
all
of
the
city
departments,
city
partnerships,
stakeholders,
engage
all
of
our
constituencies,
leverage
our
expertise,
leverage
our
partnerships.

is
an
opportunity
for
collective
action
and,
of
course,
there
is
our
mutual
desire
for
aligning
our
values
and
our
priorities.

timeline

are
the

reports
generated
in
alternate
years?
