CONSENT ITEM --- Item 1 is continued from the November 1, 2012, November 15, 2012, and December 6, 2012 meetings of the Commission.
1. Request from SPTIHS Property Trust (owner) d/b/a Union Park Health Services represented by David Hagerty (officer) to rezone property located at 2401 East 8th Street.
A) Determination as to whether the proposed amendment is in conformance with the Des Moines' 2020 Community Character Plan.
B) Amendment to the Des Moines' 2020 Community Character Plan to revise the future land use designation from Low Density Residential to Medium Density Residential. (21-2012-4.15)
C) Rezone from "R1-60" One-Family Low-Density Residential District to "R-3" Multiple-Family Residential District, to allow expansion of the existing legal non-conforming residential nursing care facility to provide a breezeway and entry vestibule connecting two existing wings of the building. PUBLIC HEARING ITEMS
2. Request from Eagle Iron Works (owner) 129 East Holcomb, represented by Gary Krantz, for vacation
of the remaining portions of the east/west segment of alley between Reigart Street and East Holcomb
Avenue running west of Oxford Street. (11-2012-1.25)

3. Request from Union Pacific Railroad (owner) for vacation of the following four segments of street rightof-
way where the railroad right-of-way intersects. This would result in the termination of the public
street on either side of the railroad, thereby closing it to through traffic:
A) Hull Avenue between Delaware Avenue and Dixon Street. (11-2012-1.22)
B) Scott Avenue between Southeast 20th Street and Southeast 22nd Street. (11-2012-1.23)
C) Southeast 34th Street between Scott Avenue and East Court Avenue. (11-2012-1.21)
D) Southeast 36th Street at a point north of Scott Avenue. (11-2012-1.24)
4. Request from Kent Lehs (owner) to rezone property located at 2425 Euclid Avenue.
A) Determination as to whether the proposed amendment is in conformance with the Des Moines'
2020 Community Character Plan.
B) Amendment to the Des Moines' 2020 Community Character Plan to revise the future land use
designation from Commercial: Auto-Oriented, Community Commercial to General Industrial.

C) Rezone from "C-2" General Retail and Highway Oriented Commercial District to "M-1" Light Industrial
District, to allow reuse of the former lumberyard property for warehousing and contractor storage.

5. Request from James Sarcone (purchaser) to rezone property located at 4506 Southwest 9th Street.
The subject property is owned by David Holmes.
A) Determination as to whether the proposed amendment is in conformance with the Des Moines'
2020 Community Character Plan.
B) Rezone from Limited "C-2" General Retail and Highway Oriented Commercial District to Limited "C-
2" General Retail and Highway Oriented Commercial District whereby the condition prohibiting the
use of the property for automobile establishments for display, hire, rental or sales is removed, to
allow reuse of the existing commercial building for a vehicle display lot. (ZON2012-00218)

6. Request from St. Gregory Centers Holdings, LLC (purchaser) represented by Michael Vasquez
(officer) for review and approval of a 7th Amendment to the "Airport Commerce Park West" PUD
Conceptual Plan regarding property located at 6151 Thornton Avenue, to allow conversion of 19,000
square feet of the existing 38,317-square foot building from a retail/restaurant/office use to a
rehabilitation center where 6,000 square feet is proposed for general office use, 3,000 square feet is
proposed for medical outpatient clinic use, and 10,000 square feet is proposed for a residential social
detoxification use. The subject property is owned by Valley Bank. (ZON2012-00219)

7. Request from ST Howard Development (purchaser) represented by Steve Howard (officer) for approval
of a Preliminary Minor Subdivision Plat, "4111 Tonawanda" on property located at 4111 Tonawanda
Drive, to subdivide the 2.54 acre property into a 1.79 acre parcel with existing single-family dwelling,
and a 0.78 acre parcel. The subject property is owned by Susan Muelhaupt. (13-2013-1.18)
