program

children
and
young
adults
with
special
healthcare
needs
and

the
program
started,
I
believe
I
met
you
all
in
December
and
then
in
the
other
meetings
in
February,
when
we
kind
of
talked
a
little
bit
about
the
health
department
received
the
opportunity
to
have
this
program
and
basically,
since
it's
a
new
program,
we
were
doing
out
work.

We
wanted
to
do
outreach
and
to
promote
this
program
without
within
our
community.
We
don't
have
services
right
now
because
of
whole
bit,
but
we're
trying
to
do
all
our
outreach
through
our
partners
and
the
social
media,
and
we
had.
We
had
scheduled
some
events
for
our
families,
but
due
to
the
pandemic
potential
and
some
of
the
fundings
that
we
received
for
those
events,
we
were
able
to
relocate
them
for
for
to
apply
for
a
project
called
the
Kobe
emergency
project
and
we
were
able
to
receive
funds.

we
were
able
to
get
approval
from
our
program
from
our
state
and
that's
where
we're
working
on
we're
we're
reaching
out
to
work
to
our
agency,
to
our
partners
to
see
if
they
can
refer
clients
their
patients
to
us
so
that
we
can
offer
that
help
for
them.
We've
already
been
working
with
Special
Olympics.
We
started
this
Monday
that
we
we
started
with
with
them.
We've
also
reached
out
to
Ruthie.

is
Jackie.

had
a
meeting
with
her
and
kind
of
explained
to
her
that
process,
also
with
Miss
Janet
ramones
and
also
with
with
in
this
for
another
another
project
with
me,
Sanchez
as
programming
another
event
for
our
program.
But
it's
basically
that
if
you
have
in
sorry
any
families
that
can
benefit
from
this
again,
we
receive
a
children
from
newborns
up
to
the
age
of
21,
any
type
of
disability,
and
they
can.

you
can
refer
them
to
our
program
and
we
can.
We
can
provide
that
help
for
them.

physical,
emotional,
we
have
been
seen
a
lot
of
right.
We
do
a
survey,
that's
that
funny.
We
wanted
to
do
a
survey
just
to
ask
a
little
bit
what
our
families
are
going
through
right
now
and
yes,
even
though
week
they
do
have
a
cloud.
We
know
some
of
the
families
they
have
a
special
needs
without
any
type
of
physical
disability,
but
there
most
of
them
are
going
through
a
lot
of
mental
health
issues.

think
it
was
like
an
emergency
project

to
cope.
It
were
not
a
requesting
any
type
of
documentation
from
them
like
no
I
had
parents
that
they
they
were
asking
me
they
needed
to
take
a
medical
letter

her

us
to
have
we
don't.
We
don't
need
that
for
for
our
file,
because
we're
not
making
a
need
like
many
files
we
do
and
we
do
as
what
their
their
child
is
going
through
and
and
then
and
what
they
need
right
now,
that's
under
survey,
but
we
don't.

program
since
the
program
started:
it's
basically
education,
our
program
targets
to
educate
the
community
on
what
special
health
care
healthcare
is.
We
are,
of
course,
educating
the

community
and

health
care
providers
and
on

different
aspect

inclusion
and
transition.
It's
that
that

what
our
program
is
about

provide
education
to
the
community
and
to

the
healthcare
community.

program

was

participate

the

program

work
clinic

they
have
been
getting
referrals

former

salud
program.

kind
of
started
when
we
first

out
of
the
program
in
January
and
February.

referrals

different
agencies
that
new

it
started
to
know
about
the
program.
We
have
20
clients
and

when
this
COBIT
emergency
project
started,
we
just
received
other
20

clients,
new
families,
though
we're
still
pending,
to
see
more
referrals
for
them
to
come.
Of
course,
our
families
are
also
sending
other
other
families
that
they
know
through
their
support
groups,
but
what
we
we
started
since
we
started
we
we
right
now
have
up
to
40
families,

we

interact
with
patients
or
clients,
we're
doing
so
at
minimal
contacts
to
provide
those
gift
cards
or

screen

determine
eligibility
we're
tying
in
with
with
other
groups
they're
at
our
health
department
to
try
to
minimize
interaction
and
for
social
distancing.
