female
victim
later
identified

Gabriella
Sanchez
date
of
birth
of
7-2
2003
up
and
she
was
able
to
walk
outside
to
a
bench.

was
met
by
offices,
Boston,
EMS
and
Boston.
Fire
were
also
on
on
scene
Sanchez,
who
was
responsible
and
able
to
sit
up
with
assistance,
was
unable
to
keep
her
eyes
open
and
was
very
sluggish.

friend
who
was
with

stated
that
someone
was
passing

a
blunt
in

conducted

on
the
establishment
and
issued
citation
number

zero.

woman

being
escorted

onto
the
sidewalk
and

outside
the
venue.

EMS
and
police
arrived.

floor

people

security

specific

was
how

people

yeah,
normally
on
nights
like
this,
we
have
one
security
guard
standard
on
the
sort
of
a
landing
for
the
staircase,
that's
keeping
the
stairs
clear
and
also
keeping
an
eye
on
the
floor.
We
also
have
guards
positioned
as
sort
of
bookends
on
either
end
of
the
stage,
keeping
an
eye
on
that
the
center
dance
floor,
and
then
we
have
the
roamers
as
well,
so
usually
two
two
to
three
roamers
at
a
time
walking
through
the
area
trying
to
keep
an
eye
on
things.

this

do
observe
smoking

do
our
best
to
contain
it.

it's
difficult
and
the
crowd

lot
of
people
are
lighting
up
and

arrival,
the
security
team
of
the
power
points
to
the
location
where
the
victim
of
the
victim
at
the
bottom
of
the
stairs
near
the
bathroom
the
victim
identified,
was
identified
as
Megan
Michael,
who
date
of
birth
of
September
13
2001..

was
conscious
at
the
time
of
interacting
with
all
medical
personnel
and

able

speak
coherently

cut
and
was
bleeding

her
forehead
on
the

told
by
friends
that
she

tripped

of
her
friend's
legs,
but
this
one
of
the
security
staff
stated
that
she
had
fell

upstairs
to

stretch
and
taken
to
both
his
little

Hospital
Sergeant
McLaughlin
responded
and
conducted
a
code,

violations

supplemental
report

from
Sergeant
McLaughlin
who
reads
his
followers
officers,
who
responded
to
a
9-1-1
call
for
a
purchase.
Some
personal
unconscious,
a

in
the
Square
on
arrival
officers
observed
the
victim
Megan
Michael,
the
Boston
University
student,
being
assisted
onto
a
structure
by
EMS
officers.

Not
not
able
to
get
a
straight
story
is
how
she'd
receives
a
cut.
The
first
story
she
had
tripped
and
another
story.
She
had
fallen
down.
The
stairs
Miss
Mike
was
having
trouble
speaking
while
she
was
in
the
ambulance.
Sergeant
McLaughlin
was
able
to
was
unable
to
tell
for
sure
if
she
was.
This
was
due
to
alcohol
consumption
or
the
the
blow
and
gas
that
she'd
received
to
her
head.

result,
a
licensed
premise
violation
was
issued
by
for

being
injured
on
the

a
violation
was

by
Sergeant
McLaughlin

McGee,

manager

scene
signed,
acknowledged
and
receipt
of
the
citation.

on
this
particular
night

exactly

bar
owners

this
issue
immediately.
They
called
9-1-1
as
soon
as
they
found
the
person
was
injured,
EMS
and
the
police
showed
up.

immediately
did
an
incident
report
which
I've
provided
to
this

this
board
the
the
woman
herself
she
was.
She
did
not
provide
any
statements.
However,
she
was
with
provided
a
statement
which
says
and
I
provided
to
you.
We
had
a
drink
each
and
while
headed
to
the
toilet,
my
friend
tripped
over
my
leg
and
missed
a
step
and
fell
I
suggest
to
you
that
what
had
happened
is
she
missed
the
steps
she
tripped
over
a
friend's
eye.

She
had
high-heeled
shoes
on
that
was
observed
by
our
security
guard
and
she
just
tripped
going
down
to
to
make
sure
that
we
that
this
was
consistent.
We
went.
We
found
that
there
was
a
recording
of
a
sale
of
two
drinks
to
her
and
her
friend
on
this
particular
night.
So
not
only
is
the
is
the
statement
from
the
witness
consistent
but
the,
but
the
backup
showing
that
she
they
did
only
have
one
drink
is
consistent.

that
this
is
just
an
accident
that
happened,
there's
no
violation
of
of
138,
and
you
know
it
was
handled
the
way
you
want
these
bar
owners
to
handle
it

the
police
and
let
them
come
in
and
do
an
incident
report

sure

you've
done
everything
on
your
end,
so
I'm
not
sure,
there's
much

parked
across
the
street
and
approached
the
establishment
officer,
Diaz
activated
his
body
Walling
camera,
as
they're
approaching
they
observed
a
line
to
enter,
had
been
formed
to
the
right
of
the
front
door.
On
the
left.
There
were
a
number
of
people
congregating,
one
of
the
staff
was
speaking
loudly
to
a
patron
whom
they
appear
to
have
just
removed
from
inside
the
bar,
as
the
officers
approached
the
front
door,
Sergeant
McLaughlin
toward
told
the
doorman
that
he
was
there
to
check
the
licenses.
He
asked
the
doorman
to
see
a
clicker
to
check.

The
number
of
people
inside
did
not
show
clicker.
Nor
did
he
look
at
one,
but
he
said
that
they
had
175
people
inside
the
sergeant
knew
from
past
visits
to
the
bar's
maximum
occupancy
is
exactly
100.

McLaughlin
McLaughlin
entered
the
establishment
with
officer
Diaz
and
immediately
began
to
inspect
and
record
the
license
numbers
when
he
finished
writing
the
license.
Premise
form
from
the
inside

doorman

Sergeant
McLaughlin
asked
him
to
see
a
clicker
said,
and
he
said
that
there
was
195
persons
in
there.

The
place
was
clearly
over

because
it
was
wall-to-wall
people,
Sergeant
McLaughlin,

that
he
had
to
have
an
accurate
count
on
the
clicker.
At
all
times,
a
licensed
premise:
violation
was
issued
for
being
overcrowded,
based
on
a
sub
on
the
Sergeant's
observations,
combined
with
multiple
numbers
that
he
were
told
by
two
different
dormant.
It
was
also
it
was
also
noted
on
the
violation.
There
was
not
a
clicker
present
at
the
door
and
the
one
they
finally
produced
was
clearly
incorrect.

Sergeant
McLaughlin
asked
to
speak
to
Mike
and
Judy
was
identified
as
Kristin
who
was
directed
me
outside,
where
I
met
where
Sergeant
question
Sergeant
McLaughlin
explained
that
they
were
indeed
needed
to
have
an
accurate
account
and
a
person
with
a
clicker
at
the
door.
At
all
times,
he
told
Kristen
that
he
was
issuing
a
violation
for
being
overcrowded
based
on
the
answers
that
he
had
been
given
and
the
observations
he
had
made.

to
her
that
it
was
a
responsibility
for
that
person
at
the
door
to
have
an
accurate
account
with
the
plugger

Sergeant
McLaughlin
that
she

She
wanted
it
written
down,
but
he
refused
to

McLaughlin

it

violation

that
the
bar
had

responsibility
to
keep
an
accurate
count,
so
the
officers
can
check
when
they
show
up
for
an
inspection,

McLaughlin

subsidious
in
his
opinion,

size

crowd,
and
he
thought
it
was
well
over
the
allowed
amount
of
175.

our

overwhelmed

person

premises

scuffle
that
had
happened.

when
the
police
came
the
person
the

who
had
the
clicker
was
not
there
and
the
doorman
tried
to
compensate
in
some
way,
we're
not
quite
sure
but
I'm
sure
the
board
that
they
have
been.
Let
go
since
the
start
of
the
new
school
year
and
as
looking
at
it
over
the
summer,
we
have
new
procedures
in
place.
The
first
procedure
is
is
that
we
now
have
three
door
men
on
the
door
as
soon
as
the
doorman
go
on
the
door.

They
go
through
the
license
premises,
they
take
their
clickers
out
and
they
take
an
accurate
count
of
the
people
already
in
there.
That's
usually
a
very
low
number,
but
we
make
sure
that
the
doorman
all
have
the
same
number
on
their
clickers
and
they're
all
doing
it.
In
addition,
we
have
gotten
headsets
for
each
of
the
door,
men
so
that
they
can
talk
to
each
other
and
that
they
know
exactly
what's
going
on
and
there
is
designated
a
head
door
man
so
that
all
the
doorman
know
who
they
are
reporting
to.

We
also
keep
a
doorman
on
the
rear
exit
of
the
license
premises.
Who
also
has
a
headset.
Nobody
is
allowed
to
enter
with
that
rear
exit,
but
that
doorman
is
on
the
headset
and
is
also
being
constantly
told
how
much
the
the
capacity
is
and
when
it's
met,
we've
instituted
a
new
program
to
make
sure
that
the
aisles
are
always
clear
and
that
we
are
able
to
get
people
in
and
out
of
the
license
premises
at
all
time.

Since
we've
put
in
this
new
procedures,
we've
had
we've
really
had
no
problem
and
it
seems
to
be
working
pretty
well
since,
since
the
students
are
back,
the
president's
at
the
door
seems
to
be
working.
The
headsets
have
totally
upped
our
communication
skills
and
we
believe
that
that
we've
we've
gotten
a
much
better
handle
on
on
this.

Flaherty
responded
to
a
radio
call
for
an
unconscious
person
at

enough
on
arrival.
At
stage
karaoke
officer
spoke
with
EMS,
who
are
transporting
victim.
Corey
Boudreau,
who
was
intoxicated
breathing,
but
not
awake
officer,
spoke
to
Mr
boudreau's
friend
John
Keith,
who
stated
that
he
believed
Boudreaux
may
be
allergic
to
fish.

then
proceeded
to
drink
alcohol
in
the
evening,
Keith
stated
that
he
believes
Boudreau
only
had
two
beers
and
two
mixed
drinks
at
the
bar
Keith
was
unsure
if
Boudreau
had
anything
to
drink
prior
to
arriving
at
the
place
before
dinner,
Mr
Pedro
was
transported
to
the
Saint
Elizabeth
Hospital
for
further
evaluation
and
the
patrol
supervisor
officer
Sergeant
McLaughlin
was
notified.

Sergeant
McLaughlin,
assisted

stage
karaoke
at

Bright
Now
for
the
purpose
of
issuing
a
license
premise:
violation
for
over
serving
a
patron
and
Corey
Woodrow
is
detailed.
In
the
original
report
on
arrival
officer,
Conley
activated
his
body,
worn
camera
Sergeant
McLaughlin
filled
out
the
violation
form
for
over
serving
a
patron,
the
manager
on
duty,
Patrick
Sweeney,
the
general
manager
on
duty
identified
himself
as
Mr
nulen
Mr.

No
one
stated
that
he
was
the
person
who
would
call
the
ambulance
for
the
victim.
As
noted
in
the
original
report,
the
victim
Boudreau
had
been
transported
from
stage
karaoke
Saint
Elizabeth
Hospital
for
passing,
passing
out
performing
a
second
song
on
stage
well.
At
district
14,
the
victim's
friend,
who
identified
himself
as
John
Keith,
came
into
district
14
and
sparked
spoke
to
Sergeant
McLaughlin.

He
said
he
thinks
Boudreau
had
a
number
of
drinks
before
arriving
at
stage.
Karaoke
Keith
said
that
when
he
got
to
Stage
karaoke
about
8
30
PM,
the
bar
gave
them
a
round
of
shots
for
Keith's
birthday.
Keith
said
that
they
also
ordered
another
round
of
shots
after
the
first
one,
according
to
Keith
Boudreau
had
an
amazing
voice
and
that
he
was
performing
a
second
song
on
stage
when
he
passed
out.
He
described
him
as
amazing
virtuoso.

He
said
the
Boudreaux
looked
like
he
stumbled
a
bit
and
that
one
of
the
bartender
said
something.
May

Boudreau
some
water
before
he
passed
out
well,
Sergeant,
McLaughlin
officer,
Conley
would
stage
karaoke

manager,
Mr

said
that
Boudreau
only
had
a
few
drinks
while
he
was
there.
Serge
McLaughlin
explained
that
he
would
have
the
opportunity
to
explain
his
side
before
the
board
of
the
hearing.

McLaughlin

cups

large
busy

what

ordering

drinks,
three
people
need
to
be

to
