it's

apology

we
appear

fact

we
did

mayor
for

informal
discussion

you're
all
aware,
a
Mid
Staffordshire,
more
conveying
bristol
hearts
candles
are
all
scandals
that
could
have
been
avoided.
Had
concerns
been
listened
to
before
Jordan
and
after
are
we
ready?
Are
we
prepared
to
accept
a
further
scandal
in
Bristol
in
one
of
the
major
hospitals?
The
public
enquiry
to
the
mid
stuff
stated
the
overview
and
scrutiny
committee
was
wholly
ineffective
as
a
scrutiny
of
the
trust.
Counselors
cannot
be
expected
to
be
experts
in
health
care.

They
can,
however,
be
expected
to
make
themselves
aware
of
and
pursue
the
concerns
of
the
public
who
have
elected
them.
That
is
surely
the
intended
purpose
have
given
a
local
scrutiny
roles
of
councillors
in
a
previous
scrutiny,
meeting
South
Gloucestershire
councillors
unanimously
voted
to
refer
bans
case
at
a
Secretary
of
State
Bristol
Council
has
unanimously
voted
to
not
refer
the
case
and
failed
to
consider
any
of
the
evidence
prevents
presented
to
them
ignoring
key
failings
with
similarities
to
the
previous
cardiac
cases.

As
a
member
of
the
public,
I
would
like
to
raise
concerns
of
any
conflicts
of
interest
within
the
committee
and
asked
for
an
explanation
why
there
was
a
total
refusal
to
a
problem
scrutinize
the
CEO
of
the
trust
by
the
Bristol
scrutiny
committee.
The
public
are
now
well
aware
of
the
potential
cover-up
of
our
son's
death
and
demand
to
see
accountability
from
both
the
board
of
directors
at
UAH
Bristol
and
the
scrutiny
Commission
there
been
over
a
hundred
public
statements
so
submitted
today.
That
cannot
be
a
must,
not
be
ignored
if
they
are
ignored.

The
next
time
there
will
probably
be
a
files
and
statement
submitted
been,
was
given
a
drug
two
days
outside
Prozac
all
which
caused
them
to
collapse.
To
have
a
cardiac
arrest
and
I
was
later
died.
Staff
left
him
with
a
bacterial
infection
for
two
days
without
antibiotics.
Development
caused
him
to
develop
sepsis.

If
then,
for
two
years
colluded
to
cover
up
his
death
to
ensure
the
reputation
of
the
trust
is
maintained.
That
has
been
collusion
among
staff,
to
remove
evidence
and
in
both
cases,
staff,
a
court
and
the
behavior
has
gone
unpunished.
Absolutely
no
accountability.
The
same
excuses
are
being
given
now,
as
that
were
given
in
the
heart
scandal,
and
it's
just
not
good
enough.
The
public
will
no
longer
tolerate
this
behavior,
as
these
are
our
children
that
the
hospital
is
looking
after.

There
has
been
no
change
at
this
hospital,
and
let
me
tell
you
why,
when
staff
failed
to
treat
appropriately,
there
is
no
accountability.
When
staff
tries
to
delete
evidence
or
sample
of
evidence,
there
is
no
accountability.
When
staff
like
a
lewd
and
conspired
to
cover
a
child's
death,
there
is
no
accountability,
the
stuff.
No,
there
is
no
consequence
to
any
kind
of
action
taken
or
fail
made.
So
therefore
they
live
in
a
bubble
where
they
believe
they
are
irreplaceable.
They're
part
of
an
appalling
culture
at
this
hospital
that
has
grown
only
a
very
eyes.

This
culture
is
not
something.
That's
just
happened
overnight.
There
is
a
rot
in
this
trough
that
is
rapidly
spreading
and
before
we
know
it,
we'll
have
another
scandal
on
our
hands.
That
makes
mid
stuff
looks
like
a
drop
in
the
ocean,
we're
sitting
on
a
time
bomb
of
the
trust,
and
it's
inevitable
that
we
will
see
a
further
scandal.
This
behavior
is
tolerated.
The
scrutiny
committee
needs
to
do
what
they
are
elected
to
do
on
behalf
of
the
public
I'm
scrutinize
and
hold
accountable
the
Board
of
Directors
at
UAH
Bristol.

is
evidence
to
show
that
the
CI
was
lied
and
it's
still
lying
to
protect
the
cover
top
story
and
the
trust
reputation.
I,
therefore
ask
that
appropriate
action
be
taken
to
scrutinize
his
current
position.
If
the
board
of
directors
at
UAH
Bristol
are
not
willing
to
carry
this
out,
then
they
also
must
be
held
accountable.
I
asked
on
behalf
of
the
public
that
you
do
everything
in
your
power
as
a
scrutiny
committee
to
prevent
a
future
tragedy
and
a
future
scandal
that
is
well
and
surely
avoidable.

understanding
is
that,
when
an
indeed
Brenda
who
was
chairing
the
committee
when
this
was
looked
at
last
year,
my
understanding
is
that
the
decision
was
made
to
refer
this
issue
to
the
health
scrutiny
in
common
meeting.
It
was
scheduled
for
February
and
that
the
advice
now
is
that,
because
of
the
significant
change
in
circumstances
and
information
that
it
would
be
appropriate
to
request
that
that
meeting
is
brought
forward
and
as
how
old
as
quickly
as
possible.

there
was
a
conflict,
so
I
will
very
specifically
ask
not
now,
but
in
response
after
the
meeting
for
a
categoric
statement
in
that
respect,
because
III,
whilst
it's
entirely
your
prerogative
to
make
comments,
I
think
it's
appropriate
that
we
do
defend
members
who
will
and
should
have
been
acting
objectively
and
without
conflict.
And
if
there
was
any
evidence
of
any
conflict,
then
we
need
to
identify
that
now
and
deal
with
it
and
ensure
that
that
problem
does
not
arise
at
the
next
meeting.

what
we
are
proposing
and-

this
board
would
want
a
report
back
from
that
meeting
and
we
would
want
the
assurances
which
we
have
absolute
confidence
will
be
there
at
the
integrity
of
that
decision-making.

concern
in
the
statement.

asked
that
we
as
a
commission
do
a
site
visit
to
inspect
my

inclination
is
that
I
don't
believe

inspect,
because
we
accept
very
much
the
public
statement
that
he's
made
that
it
is
unacceptable
to
have
bus
stops
at
a
major
transport
link
that
are
not
easily
accessible
for
those
with
the
accessibility
difficulties.

I'm
alarmed
from
what

saying

scrutiny

being

frightening

report
on
the
website,
but
I
could
only
find
buildings,
so
I
might
have
gone
to
the
wrong
section

people

thinking
slightly
about
South
need.

all
these
states
are
suffering
from
the
loss
of
their
neighbourhood
partnerships.

closure
of
rent
offices
and
the
administration
now
is
being
done
by
unsalaried
pensioners
and
I
can
give
you

one
self-made
and
whatever
I
can't
remember
and
I
think
a
total
lack
of
interest.
In
my
opinion,
in
young
people
and
I'm
talking
about
under
18
is
irresponsible
and
by
you
know
it
is
mainly
the
the
poor
who
are,
of
course,
the
majority,
and
where
do
they
have
any
intellectual
or
physical
challenges
at
the
weekends
or
in
the
school
holidays?
I
I
have
here
a
whole
page
of
a
statement
for
a
court
headed
living
in
South
Mead
and
this
poor,
desperate
mother,
her
child.
She
was
one
of
the
teenagers
involved
in
true
trouble
on
Lincoln
Square
and
she
wrote
and
then
she
said
what
really
gets
my
back
up.
His
councillors
are
quick
to
jump
on
the
bandwagon,
but
do
nothing
to
help
the
kids
have
something
to
do
other
than
hang
around.
I
Also
don't
forget
of
it
inside
instead
of
moaning,
do
something
for
kids
not
just
today,
but
future
kids
put
something
useful
in
South
Mead
and
bring
back
a
place.
That's
not
slanged
off!
Well,
it
didn't
wasn't
very
difficult
for
me
to
give
ideas
and
I
hope.
They're
not
just
rejected
out
of
hand.
For
example,
in
Germany
school
playgrounds
are
open
at
the
weekends
and
possibly
summer
evenings
Oh.
Couldn't
that
be
looked
into,
obviously,
there'll
be
lots
of
problems,
but
isn't
the
problem
of
children
having
nothing
to
do
a
bigger
problem
and
sorting
out
insurance?

I
still
think
we've
got
to
hang
on
to
our

not
believe
the
government's
austerity
agenda,
it's
absurd
economic,
ignorance
and
so
on,
and
are
you

we
were
a
real
democracy?
We'd

able

the

read

people
have

response

library
closures.
Luckily,
a
woman
gave
me
her
response
and
she
found
that
the
public
might
be
used

overview

comprised
of
South,
Gloucester,
North,
Somerset
and
Bristol,
and
we're
looking
at
the
the
STP
plan
that
the
NHS
is
currently
looking
at
implementing
all
the
CCG.
And
we
are
suggesting
that
the
current
members
of
that
committee
should
actually
become
a
standing
health
scrutiny
committee
because
we
don't
actually
have
one
at
the
moment.

risk
manager

the
council
and

me
as
colleague,
John
Stephens,
flood
risk
officer
in

same
team.
We're
here
today
to
introduce
the
update
of
the
local
flood
risk
management
strategy
and
John
will
go
through
the
presentation
to
give
you
more
detail
on
that.
The
reason
that
we're
here
we
sit
in
the
strategic
city,
transport
team
within
the
transport
service
and
whilst
the
the
duties,
the
flood
risk
related
duties
under
the
council,
fall
on
many
teams.
It's
actually
within
the
Constitution.

strategic
city
transport
team
to
undertake
the
main
duties
and
oversee
the
the
rest
of
the
council's
duties
in
terms
of
flood
risk.
So
that's
why
we're
here
today
and
you
were
sent
quite
a
lot
of
material
as
part
of
this,
and
we
had
to
send
you
that,
because
that's
the
full
suite
of
documentation
that
the
strategy
is
is
contained
within
so
there's
the
main
report
and
then,
which
is
obviously
a
long
document
that
goes
into
a
lot
of
detail.

summary
map

introduce
as
we
go

so
yeah
we'll

our
presentation

strategy
was
produced
and
adopted
in
2014,
so
that
covered
that
requirement.
As
part
of
maintaining
the
strategy,
we
go
to
regular
reviews
with
other
risk
management
authorities
that
I've
mentioned
annually
at
scrutiny.
Boards,
which
previously
was
placed
scrutiny
and
with
ourselves
this
year,
we're
also
going
through
the
formal
update
this
year,
which
is
to
incorporate
new
understanding
to
account
for
work
undertaken
since
the
strategy
inception
and
to
plan
out
future
works,
including
the
effects
of
climate
change.

I've
added
this
map
display
this
is
surface
water,
flood
risk,
which
is
a
simulation
of
a
heavy
rainfall
event
over
Bristol
and
highlights
vulnerable
areas
such
as
you
can
see
around
Ashton
we've
got
a
number
of
houses
in
one
of
the
big
blue
patches
there.
So
the
way
the
strategy
was
formed
was
we
can
focus
our
attentions
on
certain
areas
where
we
can
incorporate
flood
schemes,
units
lots
of
the
other
smaller
patches
and
which
are
quite
disparate
and
spread
out
throughout
the
city.

We
have
to
look
at
wider
changes
to
policy
planning
practice
things
like
that,
trying
to
count
for
that
and
to
make
improvements
on
surface
water
management
and
drainage.
So
the
strategy
is
all-encompassing
as
a
lot
of
variation.
Different
types
of
actions,
aspects
like
this
Community,
Engagement,
Emergency,
Response
and
so
on.

The
strategy
is
structured
in
line
with
the
Environment
Agency's
national
strategy,
but
we've
local
spin
for
Bristol.
It
has
overarching
high-level
objectives
which
are
then
broken
down
into
measures
and
then
into
specific
actions
which
form
a
strategy:
action
parent.
Therefore,
the
strategy
is
measurable,
so
we
can
monitor
our
progress
and
carry
out
particular
works.

We've
progressed
from
the
original
strategy.
We've
included
a
couple
of
the
flood
schemes
as
an
example
of
what
we've
done
in
recent
years.
So
here
we
have
a
deployable
flood
barrier
that
was
previously
erected
even
present
to
prevent
tidal
flooding
of
a
number
of
properties
along
that
road.
Since
then,
in
with
the
Metro
bus
works,
the
flood
defense
wall
on
Cumberland
Road
has
actually
provided
flood
flood
protection
from
that
now,
so
no
need
for
the
barrier
at
South
Mead
on
the
bottom
left
there
you
can
see
a
heavy
rainfall
event.

This
was
an
error
that
was
highlighted
on
one
of
our
flood
risk
studies.
So
when
we
went
out
during
a
heavy
event
to
verify
this,
it
showed
that
it
was
at
risk.
So
since
then,
we've
managed
in
a
nearby
font,
Hill
Park
to
dig
an
attenuation
Basin
to
take
the
water
that
would
usually
flow
to
that
point
and
allow
it
to
drain
to
ground
over
time
or
release
slower
through
the
catchment.
So
it
doesn't
actually
put
these
properties
at
risk.

The
map
display
that
you
would
have
received
is
what
we
intend
to
circulate,
which
highlights
some
some
of
those
sorts
of
work
and
gives
an
explanation
of
our
role
in
IMPLAN
in
insurance,
sustainable
development
and
adequate
drainage
and
new
developments
along
with
some
of
the
work.
Other
work.
Ongoing
activities
in
the
team,
such
as
water
cost
maintenance.

As
I
mentioned,
the
strategy
action
plan
makes
it
measurable
the
the
work
that
we've
done,
how
we're
progressing
and
managing
flood
risk
within
the
area.
So
we've
got
Italian
air
of
statistics
or
actions
completed,
such
as
flood
schemes
at
Dundee,
hils,
introducing
flood
prevents
in
at
bottom
two
hills
in
some
areas,
along
with
installed
and
rain
gauges,
river
monitors.

Just
for
a
brief
of
that,
we
have
some
revenue
that
covers
maintenance
such
as
the
Water
Course
maintenance,
there's
capital
funding
available
for
some
of
the
smaller
flood
mitigation
schemes,
and
then
other
projects
will
have
their
own
individual
business
case,
which
may
have
interest
from
other
parties
depending
on
the
type
of
flood
in
forest
and
sewer
flood
in
Wessex.
Water
may
get
involved
main
river
flooding
the
environments
agency.
All
of
these
business
cases
will
need
to
ensure
that
they
are
sustainable
going
into
the
future
taken
into
account
and
maintenance
cost.

then
intend
to
circulate
this
to
all
councillors,
with
an
option
for
a
briefing
session
put
this
publicly
available
as
before,
with
the
existing
strategy
on
the
council
website
and
go
out
to
our
community
contact.
So
it's
circulated
as
widely
as
possible
and
another
thing
that
we'd,
like
help
with
from
the
group,
is
reaching
out
to
other
community
groups
to
raise
awareness
of
flooding
in
communities
and
the
way
we're
thinking
we
could
do.

I've
been
on
the
council.
This
has
been
something
which
is
there's
been
one
of
the
most
important
things
that
we
have
to
contend
with,
and
for
much
that
time
there's
been

argument
between
that
bear:
a
barrier
in
the
in
the
Aven
and
individual
walls
around
sites
at
risk
in
in
the
central
Bristol,
as
if
at
around
the
paint
works,
which
was
a
massively
expensive
piece
of
piece
of
work.

can't
find
now
any
reference
to
the
barrage
in
here,
and
perhaps
you
can
help
me,
because
I
did
skim.
Some
of
this
and
I'm
sure
we're
all
aware
that
the
worst
thing
that
would
possibly
happen
in
Bristol
is
that
everything
happens
at
once,
and
the
barrier
is
is
uniquely
suitable
for
holding
back
one
of
those
one
of
those
extra
special
events
which
turns
all
the
rest
into
a
nightmare.
So
I'm
very
surprised
that
if
this
is
just
a
subtle
reworking
or
something
which
is
what
the
introduction
said,
it
was.

both
a
strategy
and
that's
one
specific
action
in
this
citywide
strategy.
So
we
weren't
gonna
go
into
the
details
of
each
specific
action
or
specific
project,
albeit
a
big
one,
similar
over
mouth
seven
side.
That's
a
very
big
piece
of
work
that
we're
undertaking
the
moment
we
weren't
going
into
the
detail
of
those
actions
in
this
meeting.

Yes,
but
both
of
these
aspects
are
picked
up
in
the
report
like
I
said
that
the
river
Aven
strategy
is
a
specific
action
and
there's
information
in
the
main
citywide
strategy
report
that
explains
tidal
flood
risk
is
a
significant
issue
to
the
city
center.
We're
dealing
with
it
through
that
specific
project
and,

meetings
was
pushed

we

end,
only
had
one

full
day

find

way

bringing
them
into
a

municipal
whatever

starting
observations.

was
that
we
accepted
the
cuts
that
had

made.

not
trying
to
roll

realized

next
year
and

stuck
with
that.

was
the
rules
as
far
as
we
saw
it.

suggest
ways
that
the
the
administration
might
reign
back
on
some
of
his
prejudices
about
how
we
could
use
volunteers
and
how
volunteers
could
eke
out
the
service
to
keep
it
in
play
long
enough
for
the
longer
term
solution

kick
in.
But
we
didn't
try
and
change
those
fundamentals,
because
that
I
think
would
have
been
inappropriate
in
terms
of
our
brief.

would
like
personally
to
say
that
I
wish
there
were
other
ways
of
doing
this
and
I
think
there
probably
are,
but
within
an
all-party
group,
with
a
very
fixed
and
short
here.

ward

great

small
foot
fall

job

managing
to

trying
to
support
in

libraries

the

closed

many

central

Central
Library

not
just
a
super
lending
library,

archival
library

something
which
has

important
legacy

for

dispose
of
that
without
a
thought
for
the
sustainability
of
the
archival
resources
is
something
we
have
to
think
about

very
carefully.

role

civic

haven't
done
is
tried

rush

to
consultation
and
put
something
together.
We've
got
a
small
number
of
proposals
which
we'll
have
surveys
open
alongside
the
budget
consultation,
but
the
actual
budget,
consultation

corporate
strategy.
Consultation
itself
is
fairly
short
and
succinct
and
keeps
to
more
traditional
questions.

get
asked
about
the
council
tax
and
social
care
precepts.

terminology
is
important

I
mean
we
keep

word

connotation,
but
there's
certainly
the
neighborhoods
one
was

hardly

was
discouraged
people
from
providing
their

they
may
not
be
aware
of
those
issues
in
the
same
way.

it's
still

you're
right
that
they
will
be
painting
a
different
picture
of

performance
criteria
is

actually
see
where
things
are
going
horribly
wrong,

team

various
members

off

leave
so

I
think
it's
a
risk
to
the
scrutiny
process
and
I'm
not
going
to
say
at
this
point

risk
of
having
turned
into
a
scrutiny
geek
the
center
for
public
scrutiny,
is,
is
running
a
conference
on
complexity,
governance
and
scrutiny
for
all
people
across
the
whole
of
the
country.

in
December
and
I
asked
about
support
for
councillors
for
training
and
I'm
a
bit
frustrated
about
not
being
able
to
find
out
how
I
access
myself
scrutiny,
training.
So
I'm
asking
for
some
training
to
develop
scrutiny,
skills
I
think
it's
really
important
that
we
keep
on
top
of.
What's
going
on
in
the
rest
of
the
country,
and
I
asked
questions
about
what
the
budget
for
resourcing
councilor
development
and
attendance
at
conferences
and
scrutiny,
events
and
I
would
like
some
clarity
about
that.

asked
several
weeks
ago
and
I
think
it's
for
the
good
of
everyone
on
this
committee
that
we
can
access
these
sorts
of
developmental
opportunities
and
make
in
some
way
perhaps
make
up
for
some
of
the
deficiencies
we've
got
in
the
system
because
you've
got
less
resource.
So
can
we
have
some
clarity
on
that
I
know
there
is
member
training,
support
budgets
and
I'm
gonna
say
now:

as
the
budget
holder,
I
have
responsibility,
I
can't
respond
to
one
individual
member
about
one
specific
piece
of
training,
so
I
advised
at
the
planning
meeting
that
there
should
be
raised
today,
which
cows
relish

helpfully
done.
If
collectively
awesome
field,
this
training
would
be
of
benefit
and
collectively.
The
committee
are
all
saying
to
me,
as
an
old
party
committee,
that
you
think
this
might
be
of
use
and
obviously
that
I
can
then
look
to
facilitate
that
happening.

the
members
steering
group,
the
member
development
steering
group,
which
is
forms
of
eight
members
from
all
parties,
is
meeting
tomorrow
and
that's
one
of
the
first
things
I
want
to
discuss
with
them
is
actually
how
we
develop
a
program
that
is
equitable
for
everybody.
However,
if,
as
a
committee,
you
were
advising
that
you
think
it
would
be
beneficial
for
members
of
azam
to
at
end
of
that
session.
I
think
that,
as
a
all
party
committee,
we
can
certainly
therefore
take
that
forward
and
look
to
set
that
up
very
easily.

I
can't
at
this
precise
moment,
respond
to
individual
requests
from
individual
members
from
individual
parties.
I
have
to
ensure
that
it's,
it
is
equitable
and
at
the
moment,
I'm
being
completely
honest,
we
don't
have
a
structure
for
that,
which
is
why
we're
meeting
with
the
steering
group
and
why
we
set
that
up
so.

That's
kind
of
my
point:
isn't
it
that
there
is
no
structure,
so
it's
impossible
to
ask
for
the
money
for
the
training,
there's
no
detail
of
what
the
resources
and
how
it
might
be
allocated-
and
you
know
it's
all
very
well
for
committee
to
meet
tomorrow,
but
time
is
running
out
for
this
particular
event.
In
any
case,
so.

recognize
the
need
for
training
and
development
across
the
board,
for
staff
and
for
members,
and
therefore,
if
we
can,
if
we
can,
try
and
broker
a
way
of
making
that
happen,
when
there
are
positive
courses
and
when
we're
going
through
this
process
have
changed.
When
we
need
members
to
want
to
develop,
then
it
would
be
helpful
if
we
can
find
a
way
of
releasing
that
budget
in
a
way.

the
purpose

why
I'm
meeting

members
tomorrow,
but
I
would
say
those

wish

are
saying
to

that
is
an
endorsement

a
cross-party
group

funding

as
a
cross-party
committee,

members

this
committee
won't
go
on
scrutiny
training.

can
facilitate
that
happening.

use
the
budget
and

support,
but
in
any
of
those
applications,
I
didn't
know
what
the
budget
is
and
therefore

wouldn't
want
to

say
to
people
should
go
on
courses
if
that
actually
might
use
up.
A
95
percent
of
the
total
members
budget
and
therefore
I
would
rather
the
mechanism
that
existed
was
applied,
but
can
I
suggest
that
Andrea
reports
back
to
the
party
group
leads
meeting
in
the
next
couple
of
weeks
so
that
the
to
the
satisfy,
if
it's
to
the
satisfaction
of
the
party
group,
leads
that
it's
been
resolved
and
is
moving
forward
and
people
are
able
to
go
on
training
courses.
We
don't
need
to
bring
it
back
to
this
meeting,
but
if
it's
not
resolved,
then
it
comes
to
this
meeting
is
an
agenda
in
December.
